Nora's Noticing Loop. Every observation becomes exactly one of three things — a block in a queue, a watchlist entry, or a question with a proposed answer. Never a silent fix. New proposals wait for you in Approvals.

The gate. Nothing enters a live queue, changes a playbook, or passes “Proposed” without your tap. Routine work you approved once (at card creation) runs silently. Safety items never expire.
